At Eventide Family Dentistry, our main goal is making sure that you are receiving the best dental care possible. During your first visit, you will be required to fill our new patient forms. These forms will take some time to fill out, so we recommend coming to your appointment about 20 minutes early. If you prefer to fill these forms out before your appointment, please refer to the following link:

Dental History

Evaluating your medical history allows our dentist to have a better understanding of your dental care needs. The more details about your dental history you provide, the easier it is for our dentist to get to the root of the problem.

Comprehensive Exam

All first-time patients undergo a comprehensive exam. A comprehensive exam allows our dentist to set a solid course of action to address any discomfort you may be experiencing. Please feel comfortable sharing any questions or concerns you may have with our dentist.


X-rays are a great way to diagnose problems that may otherwise go unnoticed. Any tooth decay, abscesses, impacted teeth or jawbone damage will show up on an x-ray. The details x-rays provide could make a big difference for your overall oral hygiene.

Treatment Plan

Once the comprehensive exam and x-rays are complete, our dentist will provide you with solutions to your dental irregularities. Through a comprehensive treatment dental plan, our dentist can resolve the issues that affect your oral hygiene. Since we develop each treatment plan based on your specific needs, it’s important that you are clear about your expectations from the start.
